Registration Form - Due January 15, 2012, with $150 deposit

Registration
Registration is on a first-come, first-served basis. Georgia has limited openings reserved for the CWF program. The initial deadline is Friday, January 15, 2012. The $150 non-refundable deposit and registration form are due at this time. If after the deadline space is still available, additional participants will gladly be accepted. To ensure your participation in CWF, register ASAP! This form must be in the State 4-H Office by January 15, 2012. Additional registration materials will be mailed after January 15, 2012.

Eligible Participants
CWF is open to all Georgia 4-H members 15 to 18 years old, as of September 1, 2012.

Cost
The standard registration package for CWF is $875 per person, plus transportation costs. Transportation costs will range between $275 and $400, depending on how many people register. Transportation costs will be determined by the number of participants, and will be announced after the final delegation size is determined. The total cost will be $1150 - $1275.

The above costs do not cover meals on the way to Washington D.C. and back. Plus, there is one day when participants must pay for their own lunch. Plan for an additional $35-$50 to cover these meals.

A $150 non-refundable deposit must be submitted to the State 4-H Office with this form by January 15, 2012 made payable to the “Georgia 4-H Foundation.” The remaining costs are due by May 15, 2012.

You may pay the cost of CWF a monthly basis or pay the total sum by May 15, 2012. If you choose to make monthly payments, the payment schedule is as follows:

Whether you choose to pay monthly or at one time, the $150 non-refundable deposit is due January 15 (along with the registration form) and the full balance must be paid by June 15, 2012.

Transportation
4-H will provide a charter bus for transportation to and from Washington, D.C., and during the week of CWF. This bus will depart Georgia from Savannah, specific location TBD. Participants must provide their own transportation to the designated site in the Savannah area.
